Include your physical or electronic signature.

If you believe content was removed or disabled in error, you may submit a counter-notice with the same formalities. On receipt of a valid counter-notice, we may share it with the original complainant and restore the content unless we receive notice of legal action within the applicable period.

In appropriate circumstances, we will terminate access for users or account holders who are determined to be repeat infringers, in accordance with the DMCA.
